In this initial consultation, we start where you here. Drawing on nearly two decades of experience across hospitals, rehabilitation settings, and outpatient medical clinics, Dr. Lauren Wolf will help you identify immediate priorities, and determine whether additional guidance would be helpful.
This conversation provides direction, answers initial questions, and is the first step toward greater clarity and a calmer path forward.

Following our initial consultation, Dr. Wolf will review your family’s situation and prepare tailored insights, resources, and potential connections that may help address the concerns you shared.
During this meeting, we explore the topics most relevant to your circumstances. This may include navigating healthcare and social services, understanding care needs, improving home or personal safety, exploring living options, or strengthening advocacy for efforts.
Whether responding to a current challenge or preventing one, this conversation focuses on practical short-term steps and informed options to support you and your loved one.

Strategy, Action, & Next Steps to: A Calmer Journey
75-minute wrap-up session
This extended meeting provides time to reflect on what has unfolded since our earlier conversations and to continue addressing the most important questions and concerns.
Together, we will review progress, clarify priorities, and focus on practical strategies that support safety, well-being, and quality of life for you or your aging loved one.
Individuals and families leave this session with a clearer understanding of available options and a thoughtful plan for the next steps ahead. If you may benefit from ongoing guidance, the benefits of longer-term care management support will be discussed.
*A one-time fee of $450 is billed for Care Clarity Sessions, following the completion of the initial meeting. Two follow-up sessions may be scheduled at your convenience, but should occur within 2 months. Care Clarity Sessions™ are available nationwide in the USA.
